Important document that should be completed by all players at the beginning of each season and included in every trainers kit. 

Goaltender Registration Credit 

Eligibility by Division

• U7 House League - No credit provided.
• U9 House League - No credit provided due to team goalie rotation.
• U11 to U18 House League - Eligible for a 50% registration credit applied toward the following season’s CGHA registration 

Details of the Goalie Incentive

1. Credit Timing
   1. Credits will be applied to the following season’s registration fee.
   2. If a goalie does not return the following season, the credit is forfeited.
2. Credit Requirements
   1. Player must be registered as a full-time goalie for the season.
   2. Player must participate in at least 80% of scheduled games and practices, except in the case of documented injury or illness.

Goalie Development Program Rebate

Families of goalies (U9-U18) who participate in the CGHA Goalie Development Program may apply for a goalie rebate. 

The rebate amount and application process will be determined annually by the CGHA Executive Committee.

• Proof of participation (attendance record or confirmation from the On-Ice Instructor) must be provided with the rebate application.

Registration Refund Request Form

To request a refund on regular season and clinic registrations. Conditions apply.

To request reimbursement of courses and clinics required by OWHA. Conditions apply.
